Repair method of failed eye bag surgery

The lower eyelid is composed of the interior tarsus, skin, muscle, fascia and fat. Its purpose is to protect the eyes. Therefore, the eye bag surgery must take the interior tarsus, skin, muscle, the laxity of fascia, and the amount of fat into consideration to avoid surgery failure. The common causes of surgery failure are as follows:

  1. Because of removing too much orbital fat or suturing the fascia of lower orbit too tightly.
  2. Lower eyelid ectropion: Mainly caused by removing too much skin, muscle or fat, or due to the laxity of lower eyelid.
  3. Lacrimal punctum ectropion (ectropion of the inner eyelid): Mainly caused by removing too much skin from the inner side, or due to the laxity of interior tarsus, resulting in ectropion of the inner eyelid and separate from the conjunctiver to cause the symptoms of epiphora.

The method of eye bag failure reparation surgery:

  1. Lower eyelid depression: Our clinic basically adopts the non-scar eye bag removal method, which can avoid such situation. If this does indeed happen, it can be solved with autologous fat transplantation.
  2. Lower eyelid ectropion: The reparation surgery mainly adopts the methods of ligamentum tarsale externum shortening and orbicularis oculi muscle suspension.
  3. Lacrimal punctum ectropion (ectropion of the inner eyelid): The surgery adopts the interior tarsus shortening method to separate the depress area of skin and its surrounding area, thereby using the surrounding orbicularis oculi muscle and subcutaneous tissues to repair the depression. Afterward, adopt ligamentum tarsale externum shortening and orbicularis oculi muscle suspension will solve the problem.

Since each person's situation is different, the specific reparation plans for failed eye bag surgeries are also different. It is recommended that you have adequate communication with an experienced surgeon before proceeding.

The gentleman had undergone the traditional eye bag surgery previously. He had been suffering after the surgery because the surgeon used the lower blepharoplasty method to remove the fat in his eye bags, and then the lacrimal groove depression and eyelid ectropion occurred, resulting in obvious lower Sanpaku symptom. This hugely affected him as he wanted to make the change without being people knowing. After being depressed for more than a year, he was introduced by his friend and came to our clinic for consultation. After the consultation, the surgeon advised him to use the non-scar eye bag removal surgery to re-arrange the internal fat, and then use the surrounding orbicularis oculi muscle and subcutaneous tissues to repair the depression. Afterward, use the ligamentum tarsale externum shortening and orbicularis oculi muscle suspension surgeries to solve the Sanpaku issue.

The above is purely a case description, and the actual situation will vary from person to person.

The lady underwent an eye bag surgery two years ago. While the surgery solved the problem with the under-eye bags, it also resulted a dry eyes issue. She thought the symptom would improve in a month or two, but that was not the case. She consulted with a number of ophthalmologists, the doctors said the problem was that the surgery caused eyelid ectropion, which in turn caused Sanpaku symptom. After consultation, the surgeon suggested her to use fascia reparation surgery, mainly adopting the ligamentum tarsale externum shortening and orbicularis oculi muscle suspension to solve the ectropion issue.

The above is purely a case description, and the actual situation will vary from person to person.
