As early as the 1960s, some European physicians had begun to use the so-called cuettage to remove body fat, which of course caused a lot of problems. By 1974, physicians began using blunt instruments to remove the human subcutaneous fat. It was not until 1982 that Dr. Yves-Gerard Illouz, a French physician, proposed the so-called "Illouz Method", using a blunt liposuction tube to remove the human subcutaneous fat with negative pressure, which yielded very good results. That was the beginning of modern liposuction technology and is still the main liposuction method at the moment.

By the 1990's, ultrasonic energy was also used to liquefy fat and became an alternative. Still,, it had to use negative pressure to aspirate the fat. After 30 years of efforts by physicians, fat cells can now be removed from the body with little bleeding, less pain and less danger.



The current surgical methods of removing fat include::


Suction-assisted Liposuction(SAL):Uses negative pressure to aspirate the fat.
Ultrasound assited Liposuction (UAL): Using a special tube to release ultrasonic energy and thereby liquefying the subcutaneous fat, and then using the negative pressure to aspirate the fat.
Power-assisted Liposuction (PAL):The special liposuction tube will make a rapid front-and-back vibration when absorbing fat, shattering the fat and helping the negative pressure to aspirate the fat.
Water-assisted Liposuction(WAL): The so-called water-assisted liposuction is that the liposuction tube will spray anesthetic water column during the course of liposuction, thereby reducing the resistance of negative pressure fat aspiration.
Laser-assisted Liposuction(LAL): It is to use the energy of laser light to destroy the fat, and then use the principle of negative pressure to aspirate the fat.


However, regardless of the method used, the biggest side effect of liposuction surgery – skin roughness – still cannot be solved. It is because that the key to a successful surgery is the surgeon, but not the instrument. Only experienced and skillful surgeons can remove the subcutaneous fat "homogeneously" from the body.


In recent years, Dr. Simeon Wall Jr. M.D, a liposuction master from Louisiana, US, has proposed the concept of the refreshing "SAFE LIPO". Although it is not a new surgical technique, it definitely can reduce the skin roughness issue after the liposuction surgery.

The so-called SAFE LIPO


"S" Separate: Firstly separate the fat cells from the blood vessel and nerves and support the tissues and fascia.
"A" Aspirate: Aspirate the separated fat.
"F" & "E" are the so-called "Fat equalization", which is to distribute the remaining subcutaneous fat evenly under the skin after fat aspiration. The seemingly simple concept of SAFE LIPO actually requires a lot of patience and carefulness to successfully complete the liposuction surgery.


For patients who have rough skin issue after undergoing liposuction surgery, we recommend using the concept of "SAFE LIPO" together with the autologous fat transplantation.


The main factor of having rough skin after liposuction is that too much fat was aspirated out locally, combined with the destroyed subcutaneous blood vessel supporting tissues and fascia, resulting in the formation of subcutaneous scar tissues and causing the regrettable results. SAFE LIPO 【Case B】

Precautions for one week prior to the surgery


  1. No smoking prior to the surgery because smoking causes vascular contraction and reduce blood supply to the skin and thereby affecting the healing of the incisions.
  2. Eat more fruits and vegetables to absorb cellulose and vitamin C, which can increase the ability of wound healing.
  3. Stop taking Aspirin and painkillers which contain Ibuprofen to avoid the postoperative bleeding issues.
  4. Limit the intake of vitamin E to under 200 mg per day to reduce postoperative bleeding issues.
  5. Control blood pressure and blood sugar: The operation must be performed under the condition of good and stable blood pressure and blood sugar.

On the day of surgery


  1. Please wear loose clothing. Do not wear high-necked clothing, and it is best to wear front-button clothing.
  2. Do not carry valuable items such as rings or necklaces.
  3. Please arrive at our clinic at least half an hour before the scheduled time of surgery.

Postoperative instructions


  1. 1.The patient needs to be accompanied by someone within 8 hours after the surgery and note any changes of consciousness.
  2. Drainage tubes will be used after the surgery to accelerate the deswelling. Therefore, the patient needs to return to our clinic the next day to change wound dressing and remove the drainage tube.
  3. Do not remove the elastic bandage yourself. If you feel unwell or if the blood leaks out from the wound dressing, contact us immediately.
  4. Always take medication on time. Take medication four times a day – after each meal and before going to bed. (If it hurts badly, take painkillers every two hours at most.)
  5. Wound care: Clean the wounds every day and night with a cotton swab moistened with saline and then apply ointment.
  6. Make return visit to our clinic one week after the surgery to remove stitches.
  7. The patient can bath, swim and massage (even applying firming cream) two weeks after the surgery.
  8. It is recommended to wear compression garments for 3 months. If it is inconvenient, wear them for at least 6 weeks to restore skin elasticity.
  9. If the liposuction site is located in the lower limbs (thigh, calf), raise the lower limbs to avoid edema and promote blood circulation.
  10. Make a return visit to the clinic one month after the surgery for postoperative follow-up.
